In the event of an emergency, I hereby give permission to the staff of Mary Queen Catholic Church to seek emergency medical transport or treatment for me. I wish to be advised before further care is given by the hospital or doctor and I understand that all financial obligations are my responsibility. In consideration of my participation in the ACTS Retreat scheduled for May 29th - June 1st, 2025 at the Christian Renewal Center (CRC) in Dickinson, TX, I agree on behalf of myself, and my heirs, successors, and assigns, to indemnify, hold harmless and defend the Daniel Cardinal DiNardo, Archdiocese of Galveston-Houston, Mary Queen Catholic Church, its pastor or any representative of the Mary Queen ACTS community (collectively the "indemnitees") from any and all injuries, losses or claims arising out of my child's participation in the program. I further agree that if anyone on my behalf makes a claim against the indemnitees, I will indemnify, save, and hold harmless each of the indemnitees from any litigation, expenses, attorney fees, loss, liability, damage or cost which may incur as a result of such claim.

The 3-day, 3-night ACTS Retreat scheduled for May 29th – June 1st, 2025 is presented by Mary Queen Parishioners. The goal of this retreat is to deepen our relationship with Jesus Christ, renew ourselves spiritually, give new meaning to our prayer life and Sunday liturgy and build lasting friendships with members of our parish. This retreat begins Thursday evening and ends Sunday with a meal of fellowship in the Main Hall at Mary Queen Church following the 11:30 a.m. Mass. Round trip transportation will be provided, leaving Thursday evening from the Mary Queen Church.

The cost per retreatant is $230.00. Registration will be on a first come first serve basis, with a waiting list thereafter. In order to reserve your place, this registration form must be completely filled out and the registration fee of $230.00 must accompany it. Completed forms can be mailed or hand delivered and will be numbered upon receipt. We will acknowledge your application shortly after it is submitted. Make checks payable to Mary Queen Catholic Church. Cancellations within two weeks of the retreat may not receive a full refund. Please note that financial difficulties should not prevent anyone from attending the retreat.
